Calibration / Recall Requirements

  • Dear Customer:

In order to give you the best service possible we ask you provide the following information.

Some of the questions is information we are required by the standard we conform to. In order for SWM to meet the requirement we need this input from you

  • If you are an existing customer you may leave this blank.
  • CALIBRATION RECALL NOTICE

SWM sends out Recall Notice each month informing you of equipment that is due for calibration. We can tailor Recall frequency to your needs.

STANDARD RECALL => Shows items that were due previous month, items do in current month, in the coming month.

MONTHLY RECALL => Shows items due in the current month.

    CUSTOMER DEFINED => Your requirements

NOTE: At any time you can call / email us for a current Recall Notice. This usually can be accomplished immediately once we receive your request.

  • DELIVER RECALL NOTICE BY:

Please indicate how you would like to have your Recall Notice sent to you. We can send email, fax or both

Note: We do not send conventional mail

  • CALIBRATION CERTIFICATES (REQUIRED INFORMATION)

    ISO 17025:2005 / ANSI Z540.X REQUIREMENT

We are required to notify the customer immediately when an instrument is received out of tolerance or not functioning properly,

SWM has always had a system in place to notify customers when this condition exist.

However because we lacked the information to report the out of tolerance report to it was rarely used.

During past audits from the accreditation body we were required to make every effort to obtain this information.

Please indicate how you wish this information is delivered to you.

  • CALIBRATION INTERVALS (REQUIRED)

It is the responsibility of the customer to provide the calibrating facility with interval of calibration.

Unless the customer provides a different interval SWM will assign an interval of 12 months. Equipment previously calibrated by SWM will use previous interval.
